"Those puffy blue bags under your eyes may not be genetics, they may be old filler." - Dr. Macrene
Having investigated this topic in detail, if you have puffy pale blue bags under your eyes, they may be due to old filler from suboptimal injections dating back many years. Do not be quick to blame your parents or allergies; it is worth allowing your top dermatologist injector (such as myself) assess and make a determination.
What causes the blue discoloration?
There are several effects of light interacting with substances that can confer a blue hue. Tyndall effect, Raleigh scattering and Mei scattering all pertain to how light interacts with particles; however, they differ in the size of the particles and the physics of scattering. Tyndall occurs with particles in a colloid, which is a gel-like substance containing large particles; Raleigh occurs when particles are smaller than the wavelength of light, as in air molecules; and Mei happens with particles corresponding to the wavelength of light, as in the water droplets present in clouds. Hyaluronic acid filler meets the scientific definition of a colloid. When light hits it, filler particles scatter the shorter blue and violet wavelengths, hence the blue hue. In sum, the phenomenon of blue discoloration pertaining to hyaluronic acid fillers is called the Tyndall effect.
Why does old filler get puffy?
In my theory, as your natural enzymes digest the hyaluronic acid in the filler, it draws in more water molecules, making the skin puffier over time. The reason for this is that hyaluronic acid is a humectant; this is a substance that attracts water molecules. As the the filler is very slowly but naturally dissolved over time, the surface area of available hyaluronic acid short chains increases, drawing in more water molecules.
How do I fix this?
My approach is to inject hyaluronidase very specifically and in a targeted artistic manner to dissolve the unattractive excessive filler to result in a smooth sculpted outcome.
